                          CASE HISTORY
Patient Details:
A 60 years old female,Occupation-housewife  resident of miryalguda presented with 

Chief Complaints:
Decreased urine output 


History of present illness:
Patient is apparently asymptomatic before 5 yrs.firstly symptomatic with  shivering and swelling  of legs, weekness and decreased urine output.got medication since 5 yrs. 
Since 1 month patient is undergoing dialysis

History of past illness:
 Hypertension since 20 yrs
 Diabetic since 2 yrs
 Edema -swelling in legs
 No Tuberculosis 

Family history:
Patients father is Hypertensive 

Personal history:
Diet-Mixed diet
Appetite-Normal
Sleep- adequate sleep
Bowel&Bladder-irregular

General examination:
Conscious, Cooperative.
Pallor-present
Icterus-Absent
Cyanosis-Absent
Lymphadenopathy-absent 
Edema-Absent. 
Q/A 
1.may this condition lead to kidney infection
2.Is there any relation between urine output and bowel 
3.can this be the case of kindney stones
