                          CASE HISTORY
Patient Details:
A 60 years old female,Occupation-housewife  resident of miryalguda presented with 

Chief Complaints:
Decreased urine output 


History of present illness:
Patient is apparently asymptomatic before 5 yrs.firstly symptomatic with  shivering and swelling  of legs, weekness and decreased urine output.got medication since 5 yrs. 
Since 1 month patient is undergoing dialysis

History of past illness:
 Hypertension since 20 yrs
 Diabetic since 2 yrs
 Edema -swelling in legs
 No Tuberculosis 

Family history:
Patients father is Hypertensive 

Personal history:
Diet-Mixed diet
Appetite-Normal
Sleep- adequate sleep
Bowel&Bladder-irregular

General examination:
Conscious, Cooperative.
Pallor-present
Icterus-Absent
Cyanosis-Absent
Lymphadenopathy-absent 
Edema-Absent. 
Q/A 
1.may this condition lead to kidney infection
2.Is there any relation between urine output and bowel 
3.can this be the case of kindney stones

Generalized weakness, fatigueability loss of appetite weight loss. SOB grade (2-3) not cough cold no PND no orthopaedic. Fever intermittent low grade chills subsided with medication.
