A 55 years old male Occupation- weaving resident of puttapaka presented with 
Chief complains
Fluctuations in bp ,sugar and fever
History of present illness
Patient was symptomatic 
with dizziness, weakness , fever and vomiting
History of past illness
Hypertension since 10 yrs
Diabetic since a month
 Family History 
Patients father is Hypertensive
Personal History 
Diet-Mixed diet
Appetite-Normal
Sleep-No adequate sleep
Bowel&Bladder-Normal
Addiction- alcohol examination 
Conscious, Cooperative.
Patient was week
Pallor-present
Icterus-Absent
Cyanosis-Absent
Lymphadenopathy-Absent
Edema-Absent

Generalized weakness, fatigueability loss of appetite weight loss. SOB grade (2-3) not cough cold no PND no orthopaedic. Fever intermittent low grade chills subsided with medication. This is an online elog book to discuss our patients health data after taking her consent.This also reflects my patient centered online learning portfolio                                   CASE HISTORY Patient Details: A 60 years old female,Occupation-housewife  resident of miryalguda presented with  Chief Complaints: Decreased urine output  History of present illness: Patient is apparently asymptomatic before 5 yrs.firstly symptomatic with  shivering and swelling  of legs, weekness and decreased urine output.got medication since 5 yrs.  Since 1 month patient is undergoing dialysis History of past illness:  Hypertension since 20 yrs  Diabetic since 2 yrs  Edema -swelling in legs  No Tuberculosis  Family history: Patients father is Hypertensive  Personal history: Diet-Mixed diet Appetite-Normal Sleep- adequate sleep Bowel&Bladder-irregular General examination: Conscious, Cooperative.
